CAARE Clients

| Case Management |
| When prospective clients
come through our doors they often feel a sense of despair, anger and isolation. The case manager offers the client strict confidentiality and serves as a professional mentor assisting the client in gaining back his/her self esteem, dignity, independence and a healthy life style. The case manager does that by meeting privately with each client to assess his/her problems and design a plan of care tailored to meet the client's individual needs. |
CAARE's Case Management services include: · Comprehensive needs assessment and evaluation · Care Plan development · Linkages to healthcare services · Linkages to psycho-social support services · Referrals to community resources · Housing referrals/placement · Volunteer opportunities · At-risk behavior education and intervention |
