Case Management
 
When prospective clients come through our
doors they often feel a sense of despair, anger
and isolation. The case manager offers the client
strict confidentiality and serves as a professional
mentor assisting the client in gaining back his/her
self esteem, dignity, independence and a healthy
life style. The case manager does that by meeting
privately with each client to assess his/her
problems and design a plan of care tailored to
meet the client's individual needs.

CAARE's Case Management services include:


· Comprehensive needs assessment and evaluation
· Care Plan development
· Linkages to healthcare services
· Linkages to psycho-social support services
· Referrals to community resources
· Housing referrals/placement
· Volunteer opportunities
· At-risk behavior education and intervention
 
Who is eligible for Case Management services?

Individuals diagnosed with HIV/AIDS who live in North
Carolina with documentation of:

· Confidential positive HIV results
· A physician's statement
· Hospital discharge statement or other medical report which verifies diagnosis
· Medical prescriptions of AZT, ddl, or ddC
· Copy of approval for participation in the state's HIV Medications program
 
How often does a client need to meet with a case manager?

The client meets with a case manager according to
his/her individual needs to identify any current or
potential problem that may arise throughout the
coordination of care.
 
Does Case Management cost anything?

No. Case management services are free to the client
and are billed directly to Medicaid, Ryan White or
Cap-Aids.
