Transitional Housing Operating Policies and Procedures


Policy Statment

CAARE is dedicated to providing housing for persons living with HIV/AIDS. CAARE will
work to ensure equal housing opportunities for all prospective residents regardless of race,
color, gender, religion, creed, national origin, sexual orientation, or familial status.

Program Statement

The house of CAARE is temporary, transitional living facilities design to promote the
development of skills necessary for independent living. Residents share common areas
such as bathrooms, kitchen, dining area, and living room. Length of residency is determined
by individual need and reassessed in accordance which each resident's program agreement.
Residents are expected to work with a case manager in the delivery of an individualized care
plan to include search for a permanent housing arrangement.

Eligibility Requirements

1. The Individual must be homeless or imminently homeless.
2. At the time of referral, each person must submit written certification by a licensed physician, stating that the individual meets the Center for Disease Control's definition of HIV/AIDS.
3. All referrals will be expected to establish consistent medical care and maintain compliance with medical treatment.
4. Referrals will be expected to establish their need for housing and eligibility for public assistance by completing an intake interview. Case management services are provided and residents will be expectedd to work with their case manager to develop and implement an individualized care plan.
5. The potential resident must be willing and able to live cooperatively in a group living situation and to abide by the policies.
6. The applicant must sign the Program Agreement prior to acceptance in the housing program.
7. The applicant must sign a Release of Information form permitting CAARE staff to work with all other service providers to obtain any verification required in order to provide transitional housing.
8. A potential resident will not be denied housing on a basis of a substance abuse history. However, the applicant will be required to submit to random drug screening at CAARE's request. Any person refusing to be tested or who tests positively will be denied residency at that time or subject to termination from the program if a current resident. The applicant will be required to contract with their case manager to continue and comply with a treatment plan that addresses substance abuse.
9. An applicant with a history of mental illness must provide documentation from his mental health provider stating his appropriateness for a transitional living program. If active treatment is recommended, the person will be expected to participate in treatment (including medication compliance) as a contingency of acceptance and continuing eligibility for residency.
10. Any applicant with a criminal record must be evaluated by a professional knowledgeable in the criminal justice system, designated by a case manager, to determine the person's suitability for residency in a transitional housing program.
11. Each applicant must be ambulatory and physically able to clothe, feed, and care for himself at the time of admission.
